Community Support Team

Integrated community mental health support services with seamless, non-duplicated components comprised of Partial Care, Supported Employment, Supportive Housing and Integrated Case Management Services, and Homeless Outreach services. Program staff members ensure that services are well coordinated with one intake assessment and one recovery plan. Services are available on call 24/7.

Partial Care – rehabilitation day program for adults experiencing psychiatric illnesses in which participants can gain the skills, knowledge and supports needed to rejoin the community in meaningful roles. The program offers opportunities to improve social and vocational skills, to learn to manage psychiatric illness and identify supports and resources that will give persons served the best opportunity to recover from their illnesses and reach their goals.

Supported Employment – career counseling, skills assessment, job development and job coaching services for adults with psychiatric disabilities who want help with their employment plans. Services are available for those whose work attempts have been delayed or interrupted due to mental illness and include:
  • Skills and qualifications assessment
  • Career counseling
  • Resume, application and interview preparation
  • Assistance with Social Security and other benefits
  • Educational goals
  • Job coaching and support
  • indicated
Supportive Housing Services – provides the opportunity and support to live in more independent, less restrictive community settings. Assistance with locating, securing and maintaining housing, supportive counseling, daily living skill teaching and linkage with supports in the community.

Integrated Case Management Services (ICMS) – available to Sussex County residents who are discharged from a State or County psychiatric hospital or those with repeated admissions to Short Term Care Facilities or local Inpatient Units. ICMS provides community support to persons served, advocates for services and monitors service provision to maximize independent living.

Homeless Outreach Services – available for people with a history of severe and persistent mental illness who are homeless or at imminent risk of becoming homeless. Focus on securing permanent, affordable, safe housing and linkage to additional program and community services as needed.
