Supportive Housing

We began providing Supportive Housing services in Union and Hunterdon Counties in 2000. The program assists people who want to live independently with supports. Supportive housing helps persons receiving services to gain access to affordable, permanent housing, link them to employment and educational programs and with community supports, and decrease their hospitalization rate. The strategy of matching affordable housing with home/community-based services is proving very effective. For those who have obtained a place to call home we have seen positive outcomes for career development, community integration and re-hospitalization reduction. Improved outcomes in the future will be realized as people adjust to new living environments, become more focused on goals for the future and staff is able to devote time to work with them on their on rehabilitation goals.

Services Include:
  • Housing search assistance
  • Moving in services
  • Mental health/medication education
  • Employment services
  • Co-occurring Disorder Services as needed
  • Supportive counseling
  • 24 hour, 7 day on call support
Services will be provided off-site in the person's living environment and will be on going, with no time limits.

Eligibility Criteria:
  • A diagnosis of a major mental illness
  • People who express an interest in independent living
  • People who have some understanding of their own mental illness
  • Union and Hunterdon County Residents
